Working closely with the Service Leader and the Support Worker team, you will be responsible for supporting a gentleman in his late 60s
The individual has dementia, diabetes, and a mild learning disability and is supported to live as independently as possible within his own home. The role involves providing person-centred support with daily living activities, promoting independence, and enabling access to local community activities and services.

UK Driving License Required? Full UK Manual Drivers Licence essential
What You'll Be Doing
You’ll be supporting 1 Gentlemen to live his daily life in the way they wants to live it. This means you will provide support in a wide variety of ways, including:
Supporting individuals with personal care and mobility
Helping people access the community and enjoy everyday activities
Medication admin
Driving our team vehicle to outings and appointments (that’s why a Full UK Driving Licence is essential)
Preparing meals and drinks that meet a variety of dietary needs
Creating a clean, friendly, and safe home environment
